Akwesasne SCOPE Project Manager at GCWP-IH
Overview:
The Mohawk Nation of Akwesasne and the Centre for Wise Practices in Indigenous Health (GCWP-IH) at Women’s College Hospital (WCH) has partnered in a project to expand and support primary health care services in Akwesasne. What started as a pilot project, funded by Seamless Care Optimizing the Patient Experience (SCOPE), has now evolved into a long-term project, having received extended SCOPE funding. The community’s healthcare needs are well determined by Akwesasne’s Medical Director and lead physician, working closely with the Project Manager at GCWP-IH to co-create project plans and implementation strategies. This work also involves partnership with the Akwesasne Department of Health and development of strong partnerships with appropriate community organizations and Indigenous community, as well as cross-sectional teams within WCH to facilitate specialist services to Akwesasne.
Hours: This project involves 2.5 days per week of work through the GCWP-IH.
Reporting to: GCWP-IH Strategic Lead.
Role Description:
The Project Manager role includes the following main objectives:
Relationship building and maintenance of current partnerships and strong relations.
Centralizing and creating culturally safe frameworks and processes.
Identifying and implementing needed supports for the Akwesasne medical clinics and Akwesasne physicians. This may include hiring processes for support staff and partnering with various Akwesasne health department Program Leads to expand delivery of care.
Identifying barriers to delivery of care and employing innovative problem solving to improve clinical processes and delivery of care, using research-based evidence.
Supporting initiatives in Akwesasne that relate to improvements in services available.
Coordinate specialist care teams from WCH to Akwesasne and provide support such as logistical work, some admin work, and development of protocols and pathways for best patient care and management of provider needs.
Coordinate funding for WCH specialists with Akwesasne administrative support, as well as all funding streams and budgets.
Additional Responsibilities:
Cultural safety and humility standards are central to this role. The Indigenous Project Manager bridges research, program design, and clinical applications through an Indigenous lens. Bridging research and project partners with Indigenous partners through CWP-IH includes foundational steps and considerations. This includes establishing relations with Indigenous partners and allies and maintaining strong relationships with the Akwesasne band counsel, departments, and community members in a culturally appropriate and humble way.
Program design and development is done in accordance with Indigenous governance framework, with promotion of Indigenous/Mohawk pedagogies and ways of being. Understanding of Indigenous data sovereignty, Indigenous knowledge translation (as well as research/program development/clinical) and working through a lens of community-based equitable approaches is foundational.
Clinical experience and community healthcare/ medical knowledge are assets. As is experience with virtual care models and electronic medical records.
Ensuring continuity of program development with best practice models, professional ethics, and standards of practice. Knowledge of Canadian health care policy and framework, health care law and legislation, patient safety, quality health care delivery and clinical applications is an asset.
